Pharmacy Services

If you receive a prescription as a result of the Service, you may select the pharmacy assigned by Company to ship your prescription. You give Company consent to send and disclose to the contracted pharmacy all information provided by you, your healthcare records, and other applicable healthcare information and personal information so that you may receive pharmacy services. You may also wish to have your prescription sent to a pharmacy of your choosing. You acknowledge that any prescriptions sent by Company’s contracted pharmacy are intended only for adults and are to be kept out of the reach of children.

Payments
